I give consent for my child to take part in the school show in the Mill Theatre Dundrum on the 14th June. I give permission for my child to be filmed/photographed as part of the event.
I agree to pay a participation fee of €40 (this fee covers all show costs including costume rental and professional filming). A downloadable copy of the show will be provided to all participants at no additional cost.
